IMPORTANT INFORMATION – PLEASE READ COMPLETELY

• Health Requirements: All dogs must have current distemper, parvo and rabies vaccinations, bordatella is recommended but not required. Puppies must have at least one set of puppy shots. All dogs and puppies must have been in your home without illness for the two weeks prior to class. Copies of vaccination receipts or verbal confirmation from your veterinarian will be accepted. Dogs who show signs of injury or are severely overweight will not be allowed to participate in agility classes until they have been release to do so by a qualified Veterinarian.
• Training Services Provided: I understand that the trainers at fetch sam! (Trainers), will work directly with me and my pet to impart contemporary animal behavior knowledge that best fits our needs, and that successful companion pet programs depend on a combination of learned skills on the part of the pet and the owner. Trainers will make every reasonable effort to help us attain goals, but makes no guarantees of performance on the part of Owner or pet as a result of providing professional dog training. Trainers, at their discretion, may refuse entry to a pet which is not healthy, is aggressive, or does not seem suitable for the services provided.
• Waiver, Assumption of Risk, and Agreement to Hold Harmless: I understand that participation in dog training is not without risk to myself and my dog as well as any family members or guests who might attend. I hereby waive and release Susanne Cohen, her Agents, fetch sam!, and the owner of the facility from any and all liability of any nature, for injury or damage suffered as a result of my participation in any class or private lesson, including any damage or injury resulting from the action of any person or dog, or for any accident occurring on the premises or its surroundings. I agree to accept full responsibility for my actions, for the actions of my dog, and the actions of my family or guests.
• Covid 19: I warrant and represent that I will remain aware of and abide by local law and public guidance on reducing COVID-19 exposure, and will follow best practices in relation to fetch sam!’s services regarding social distancing and sanitation, including wearing a mask, social distancing from people, washing hands often and sanitizing surfaces I have touched. I understand that I may be asked to confirm my health and the health of all other members of my household, including awareness of any fevers or other symptoms, before each in-person training session. I represent and warrant that I will provide full and complete, accurate information as known to me.
• Payment: Payment in full is required to reserve your space in class. There will be a $25 fee for any returned checks.
